Telehealth Consent
Before you receive care, your care team asks you to consent to telehealth. This page explains what that consent covers and who it is with, and links to the current consent document from your care team.

What telehealth consent is
Telehealth is medical care delivered remotely, through video, phone, or secure messaging, rather than in person. Before a clinician treats you through telehealth, you are asked to read and agree to a consent form.
The consent explains how telehealth works, its benefits and limits, how your information is handled, and what to do in an emergency. It also covers specific treatments, such as compounded medications and at-home lab testing, that need their own consent.
Who the consent is with
Your consent is with OpenLoop Healthcare Partners, PC and its affiliated professional entities, and their independent licensed clinicians. Atlas Health LLC is not a party to it. Atlas provides non-clinical membership services and does not deliver medical care.
The consent document is written and maintained by your care team, and they control its wording.
When and how you give consent
You are asked to review and accept the telehealth consent during intake, before a clinician reviews your case. You may be asked to confirm it again if the document changes or if you start a new treatment. Accepting the consent does not guarantee treatment. Your care team still decides whether telehealth is appropriate for you.

If the version you accept during intake differs from the version at that link, the version you accepted governs your care.
Emergencies and the limits of telehealth
Telehealth is not for emergencies. If you think you are having a medical emergency, call 911 or go to the nearest emergency room. Telehealth may not be appropriate for every condition. Your clinician may decide that you need in-person care and refer you.
